Terms of Service for Rock Chapel Park Youth Association (RCP)
Player/Parent Contract and Waiver Notice Consent: I, the parent/guardian of the registrant, a minor, believing this program to be beneficial to him/her, do hereby give my consent for him/her to be a member of the team to which he/she is assigned.
Not For Profit Organization: I realize that this association is a non-profit organization and the sports programs are carried on by volunteers. Therefore, I further agree to give support and assistance to his/her teammates and coaches as my time allows.
Sportsmanship: I accept that at all times in the park or on playing fields, players, spectators and coaches alike will encourage good sportsmanship and conduct themselves in an exemplary manner. I acknowledge that failure to do so will result in my/my child's immediate removal from the field of play and possible banning from the Park.
Waiver and Release: Recognizing the possibility of physical injury associated with sports activities and in consideration of RCPYA accepting this registrant for sports programs and activities, I hereby waive, release and discharge the Association, it's Officers, all persons as its agents, any affiliated organizations, sponsors, their employees or associated personnel, and owners of the fields or facilities utilized for its programs, against any claims growing out of or resulting from injury to him/her as a result of this registrant's participation in any RCPYA programs, and/or being transported to or from the same, which transportation I hereby authorize.
Eligibility: I also understand that a completed contract and any appropriate fee must be received by RCPYA before he/she is eligible to participate.
Refund Policy: No refund will be given after April 1, 2009. A $30 processing fee will be deducted from all refunds prior to that date.
Park Rules. Everyone is required to clean up their trash so we do not have to hire people for this purpose. As a parent/guardian I understand that I am responsible for my child's behavior and for any equipment loaned to me or my child. Parking and driving inside the park is "strictly Prohibited" while practice and games are in session. Members need consent of the RCP Board to cookout in the park.
Consent For Medical Treatment
I hereby grant permission to the adult manager, coach and business manager of the team to obtain medical care from any licensed physician, hospital or medical clinic, for the player named herein at such time as either parent or legal guardian cannot be contacted in person or by telephone. This authorization shall include all league activities, including the period required to travel to and from those activities, and we do hereby waive, release, absolve, indemnify and agree to hold harmless Rock Chapel Park Youth Association, Inc., the organizers, supervisors, participants and persons transporting the player to and from those activities, for any claim arising out of an injury to the player.
Photo Release: I hereby grant permission to Rock Chapel Park Youth Association Inc. in Dekalb County GA. to use my or my child(s) photograph on its Web site or in other official Rock Chapel Park printed publications without further consideration, and I acknowledge that RCP may choose not to use my photo at this time, but may do so at its own discretion at a later date. I also understand that once an image is posted on Rock Chapel Park's web site, the image can be downloaded by any computer user on the internet. Therefore, I agree to indemnify and hold harmless from any claims the following: RCP Board of Directors, at Rock Chapel Park in Dekalb County GA.;All Employees or Volunteers of Rock Chapel Park in Dekalb County GA.;Dekalb County GA.; State of Georgia.
This contract is subject to the rules and policies of Rock Chapel Park Youth Association, Inc. as applicable. I agree to abide by those rules, policies and guidelines.
